You are welcome to register online to book your appointment.
Complete the online registration in 7 steps: enter your full name, email address and telephone number, and we will call you back or send you an email to confirm your appointment. Choose a convenient location, date and book your appointment. Online registration is only available for paid commercial appointments, not for NFZ. NFZ registration only by phone or in person at the clinic in Tarchomin.
